To the Editor: Many people with schizophrenia exhibit poor insight into and lack of acceptance of their illness (1,2). The absence of objective proof of this disease contributes to this problem. Investigators have been interested in developing a blood-based biomarker test to help identify a biological signature to aid in diagnosis of schizophrenia, particularly in the prodrome of the illness (3,4).
Such a test has recently been developed, validated, and marketed in the United States. This test, VeriPsych, uses a multiplex immunoassay technology that profiles a proprietary disease signature comprising 51 analytes. The test has been found to be 83% sensitive and specific in distinguishing persons with schizophrenia from those without the disorder; the study in which the test's validity was established included more than 500 patients in the development phase and more than 800 patients in the validation phase (5). However, little has been subsequently published about this test, and its clinical utility remains unclear.
Our treatment team administered the test to a 30-year-old woman with a five-year history of schizophrenia who did not accept the fact that she had schizophrenia and who had problems remaining adherent to antipsychotic medications. The test yielded a positive result for the diagnosis of schizophrenia (score of –9199), with a conditional probability score that indicated a 95% chance of having the illness. After being informed of these results, our patient began to accept that her symptoms could be attributed to schizophrenia. For the first time, she expressed interest in learning about schizophrenia, acknowledged a personal connection to her family history of the disorder, and accepted her need of antipsychotic medications. Her therapeutic alliance with her psychiatrist and treatment team has improved, she has enrolled in a college-level class, and her attitude toward antipsychotic treatment remains positive. Her psychiatrist notes that this remarkable change was the result of her confidence in the blood test results—for the first time she had objective data about her illness.
Improving insight and illness acceptance among patients with schizophrenia has proven to be no small task. Psychoeducation, meta-cognitive training, cognitive therapy, and family intervention aimed at developing an understanding and acceptance of schizophrenia have shown only some success (1). Our interest in the test was not to confirm a clinical diagnosis of schizophrenia but to help a patient with schizophrenia accept her illness. We feel that if this test or future serum tests are more widely utilized, then their use as an aid in helping people who have no illness insight or who lack acceptance of their illness may be an important application. We hope that our letter will bring attention to this new test and encourage further research to replicate and validate our results. We also need to develop a better understanding of the clinical and research uses of this blood-based diagnostic aid and of its potential role and place in the treatment of patients with schizophrenia.

Each year 1.3 million patients in the United States and 2 million patients in Europe present with early signs that could be schizophrenia. Signs of schizophrenia, which include hallucinations, confusion, delusions and cognitive deficits, are often indistinguishable from those of other mental health or central nervous system disorders, presenting frequent diagnostic challenges.
Diagnosis of schizophrenia is typically accomplished through the clinician’s evaluation of symptoms. If left untreated or improperly treated, schizophrenia can lead to worsening patient conditions and ultimately poor outcomes. Neuropsychiatric conditions, including schizophrenia, are a leading cause of disability, accounting for approximately one third of years lost to disability among people aged 15 years and over.

Serum S100B: A Potential Biomarker for Suicidality in Adolescents?

Abstract
Studies have shown that patients suffering from depression or schizophrenia often have immunological alterations that can be detected in the blood. Others reported a possible link between inflammation, a microgliosis and the blood-brain barrier (BBB) in suicidal patients. Serum S100B is a marker of BBB function commonly used to study cerebrovascular wall function.
Methods
We measured levels of S100B in serum of 40 adolescents with acute psychosis, 24 adolescents with mood disorders and 20 healthy controls. Patients were diagnosed according to DSM-IV TR criteria. We evaluated suicidal ideation using the suicidality subscale of the Brief Psychiatric Rating Scale for Children (BPRS-C).
Results
Serum S100B levels were significantly higher (p<0.05) and correlated to severity of suicidal ideation in patients with psychosis or mood disorders, independent of psychiatric diagnosis. Patients with a BPRS-C suicidality subscores of 1–4 (low suicidality) had mean serum S100B values +/− SEM of 0.152+/−0.020 ng/mL (n = 34) compared to those with BPRS-C suicidality subscores of 5–7 (high suicidality) with a mean of 0.354+/−0.044 ng/mL (n = 30). This difference was statistically significant (p<0.05).
Conclusion
Our data support the use of S100B as an adjunctive biomarker to assess suicidal risk in patients with mood disorders or schizophrenia.

New blood-test to aid in schizophrenia diagnosis

A new blood-test resulting from research at Cambridge University could aid in the early diagnosis of recent-onset schizophrenia – complementing the traditional patient interview-based diagnosis.
VeriPsych, developed by Professor Sabine Bahn, Director of the Cambridge Centre for Neuropsychiatric Research, and collaborators at Psynova Neurotech and Rules-Based Medicine, is based upon a blood-based biomarker test - where characteristics are objectively measured and evaluated as an indicator of the presence of disease.
Professor Sabine Bahn, said: "Schizophrenia is a complicated and challenging disease, yet current diagnostic approaches continue to be based on patient interviews and a subjective assessment of clinical symptoms. We expect VeriPsych to be used as an aid to this current process, and we hope it will provide the psychiatrist with additional confidence in their evaluation, as well as speed up the process."
Schizophrenia is a disease that typically begins in early adulthood; between the ages of 15 and 25, affecting 51 million people worldwide with more than 250,000 cases in Britain. Current diagnosis of Schizophrenia is conducted through interviews by a psychiatrist, who follows structured guidelines such as The Diagnostic and Statistical Manual of Mental Disorders, a process that can take several months.
During VeriPsych's developmental stage, researchers analysed 200 biomarker candidates individually and in combination to assess their connection to schizophrenia. They discovered a set of 51 biomarkers with linkages to schizophrenia and to various biochemical pathways, including inflammation, metabolism and cell-to-cell signaling. In a subsequent study, the 51 biomarkers were validated in combination with a mathematical process to separate schizophrenia patients from normal controls. The study included analysis of more than 800 well-characterised blood samples collected by leading psychiatric hospitals.
The research was published in the online journal Biomarker Insights. Commercial rights resulting from this research are held by Psynova Neurotech Ltd., which was launched by Cambridge Enterprise in 2005 and is now majority-owned by Rules-Based Medicine, Inc. Professor Bahn is the Co-Founder, Director, and Chief Scientific Officer of Psynova.

Blood test for schizophrenia could be ready this year
A January 20th media release from the American Chemical Society:A blood test for diagnosing schizophrenia — the most serious form of mental illness — could be available this year, according to an article in the current issue of Chemical & Engineering News, ACS' weekly newsmagazine. The disorder, with symptoms that can include hallucinations and delusional thoughts, affects more than two million people in the United States and millions more worldwide.
C&EN Senior Editor Celia Henry Arnaud mentions the test as one part of a much broader discussion of how scientists are using non-brain cells to study schizophrenia in an attempt to speed the identification of biomarkers of the disease and develop new diagnostic tests. She notes that schizophrenia does not just involve the brain, but also abnormal levels of certain proteins that appear in other parts of the body. The article highlights groundbreaking research by a group of scientists in the United Kingdom indicating that 40 percent of the chemical changes in the brains of schizophrenia patients also occur in other body parts. The U.K. scientists are studying these biomarkers in the skin, immune cells, and blood of patients to provide a real-time picture of the disease. Most previous studies, in contrast, were done with brain tissue taken from patients after death, the article notes.
The scientists have already identified several schizophrenia biomarkers in the blood and are working with a company that plans to launch a blood test for diagnosing schizophrenia in 2010. The test could help confirm diagnoses made on the basis of psychiatric evaluations and allow earlier diagnosis so that patients can be treated earlier.###

Blood tests may reveal psychosis

By Shari RoanScientists are moving closer to developing blood tests that can diagnose serious mental disorders, according to a study published this week in the journal Molecular Psychiatry.
A consortium of researchers showed they could identify blood biomarkers for two key psychotic symptoms: hallucinations and delusions. They relied on a technique termed convergent functional genomics that integrates several independent lines of evidence from human and animal models to identify and prioritize findings. The same approach has been used to look for genes and gene pathways linked to bipolar disorder, alcoholism and schizophrenia. Last year, the researchers published blood biomarkers for mood disorders.
Much more work will be required in order for such blood tests to be used for diagnosing and treating mental illnesses. However, the technology would be a milestone because psychiatric disorders now are diagnosed -- often with a great deal of uncertainty -- by evaluating symptoms. And there are few good ways to measure, biologically, how a medication is working. The study was conducted at the Indiana University School of Medicine and Roudebush VA Medical Center; Scripps Research Institute at UC San Diego; and State University of New York, Syracuse.
"Objective blood biomarkers for illness state and treatment responses would make a significant difference in our ability to assess and treat patients with psychiatric disorders, eliminating subjectivity and reliance on patient's self-report of symptoms," the authors wrote in the paper.
Perhaps such tests would even prevent health insurance companies from refusing to pay for treatments for mental illness.
